We are recruiting a Band 7 Specialist Community Palliative Care Social Worker on a fixed‑term basis of 6 months for our community palliative care team in Camden and Islington.

Job overview

This role requires resilience, motivation and compassion to support palliative care patients and their families. You will work autonomously in a multi‑disciplinary team, managing a caseload and coordinating care to achieve the best outcomes.

Main duties and responsibilities

Clinical Responsibilities

  • Be professionally and legally accountable for all work, adhering to the standards and guidelines of the palliative care service, the Trusts, and Social Work England regulations.
  • Work autonomously as a specialist social worker and a full member of the multidisciplinary team, providing service in the community setting.
  • Contribute an understanding of the psychological, emotional, social and practical problems of individuals and families facing terminal illness and bereavement.
  • Develop appropriate plans of care and share these with the MDT.
  • Integrate cultural, religious and spiritual influences on patients and families.
  • Manage referrals from colleagues within an agreed time frame and participate in MDT discussions.
  • Identify and assess risks to vulnerable patients and families and liaise with other agencies.
  • Act promptly on safeguarding concerns involving vulnerable adults, children or carers.
  • Advise and assist patients and families in accessing community care services, benefits and financial assistance.
  • Develop and maintain therapeutic relationships, assess strengths and coping skills, support end‑of‑life planning.
  • Use information technology systems to prepare reports, maintain case records and keep statistics.
  • Assess bereavement needs and offer appropriate support or referral.
  • Work within health and safety and equal opportunities policies, protecting vulnerable adults and children.
  • Work autonomously in diverse community settings, considering socio‑economic and cultural factors.
  • Contribute to the development and implementation of clinical policies, guidelines and care pathways.
  • Uphold patient and family dignity at all times.
  • Lead responses in crisis situations, including family conflict and emergencies.
  • Participate in multidisciplinary and cross‑agency assessments.
  • Maintain accurate and up‑to‑date clinical records in the team database.
  • Provide specialist input into local health promotion and education initiatives.
  • Support the development of palliative care skills among local professionals.
  • Undertake relevant courses to expand scope of practice.
  • Maintain a portfolio of continuing professional development (CPD).
  • Ensure accurate data entry and contribute to service data collection.
  • Deliver safe, high‑quality clinical care with a commitment to continuous improvement.
  • Act as a mentor to junior colleagues.
  • Perform other duties appropriate to the post as directed.
  • Management Responsibilities

  • Use advanced time‑management skills to respond to the needs of patients, carers, staff and external agencies.
  • Support the CPD of team members, students and visitors through supervision, teaching and training.
  • Attend and actively participate in departmental, Trust and palliative care meetings.
  • Maintain effective communication within the team and with external services.
  • Assist in coordinating learner placements within the team.
  • Lead and participate in multidisciplinary meetings and case reviews.
  • Engage in personal supervision, both internally and externally.
  • Comply with all Health and Safety at Work Act policies.
  • Report equipment defects, accidents and complaints promptly.
  • Lead a project within an area of practice to improve clinical outcomes with appropriate support.
  • Stay informed about the strategic goals of the palliative care service.
