Are you passionate about promoting health equity for all? Are you curious about the fact that people living in poverty find it harder to live healthy lives and access NHS services? Do you enjoy working independently on projects, building relationships and weaving networks?
A small team of "System Convenors" has already been established at Hackney Council, working to build networks and partnerships in the borough between key organisations who support the social determinants of health. We are now piloting a Homerton Hospital‑based role who will work alongside the existing System Convenor team. The role will report directly to Annabelle Burns, Head of Integration, who will act as the senior project sponsor and will provide support and supervision for the post‑holder.
The postholder will identify and improve connections, referral pathways and processes between hospital‑based services and the voluntary and community sector for the benefit of our patients. They will gather data, insight and share learning and promote the advantages of collaboration and preventative, social determinants‑focussed work across Homerton and beyond. The role would suit someone that has established local knowledge and networks and is looking for a new challenge and opportunity to work on a time‑limited and impactful project.
