Job Description

3 month contract role with a Local Authority

Job Summary:

  • Provide professional social work support to vulnerable adults within the borough, applying a strengths‑based approach to promote independence, wellbeing and improved quality of life.
  • Undertake assessments, reviews and the development of support plans to ensure adults and their carers receive appropriate and effective services.
  • Work collaboratively with internal teams and partner organisations to manage complex cases and deliver positive safeguarding outcomes.
  • Contribute to safeguarding interventions and ensure services are delivered in line with statutory responsibilities, legislation and council policies.

Key Duties/Accountabilities (Sample):

  • Undertake comprehensive needs assessments for vulnerable adults in line with statutory duties and organisational procedures.
  • Develop, implement and review care and support plans that promote independence and wellbeing.
  • Apply strengths‑based practice to empower individuals and support them in achieving their personal outcomes.
  • Identify safeguarding concerns and take appropriate action in line with safeguarding procedures and legislation.
  • Manage a caseload of adults with varying levels of complexity.
  • Work collaboratively with multidisciplinary teams including health professionals, housing services and voluntary sector partners.
  • Support carers by assessing their needs and identifying appropriate support services.
  • Monitor and review care packages to ensure they remain appropriate and effective.
  • Maintain accurate and up-to-date case records in accordance with statutory requirements and council policies.
  • Contribute to the management of complex cases within the team and support the development of effective care solutions.

Skills/

Experience:

  • Qualified Social Worker with a minimum of 2 years post‑qualification experience (PQE).
  • Experience working in Adult Social Care with a focus on assessment and safeguarding.
  • Strong knowledge of adult safeguarding procedures and statutory responsibilities.
  • Experience completing needs assessments, risk assessments and support planning.
  • Ability to work effectively with vulnerable adults and their carers.
  • Strong analytical, assessment and decision‑making skills.
  • Experience managing complex cases within a social care environment.
  • Ability to build strong professional relationships with partner agencies.
  • Excellent written and verbal communication skills, including report writing and documentation.
  • Ability to work independently while contributing effectively to a multidisciplinary team.
  • Proficiency in case management systems and Microsoft Office applications.
