Are you passionate about ending the cycle of homelessness and ensuring vulnerable individuals transition safely into sustainable homes? We are looking for a dedicated Move-on Coordinator to join our specialist multi-disciplinary team at the Osborne Grove Assessment Centre. This is a pivotal role within our Rough Sleeping Programme, where you will take the lead on the move-on process once resident assessments are complete.
Your day-to-day will involve:
- Leading Coordination:
Taking full responsibility for move-on activity, ensuring plans are agreed upon and progressed swiftly. - Building Partnerships:
Acting as the central link between internal teams, Adult Social Care, and external partners like Supported Housing Pathway Managers and private landlords. - Unblocking Barriers:
Identifying and resolving hurdles in complex cases to ensure no resident is delayed in moving toward independence. - Collaborate with Assessment and Move-On Workers to meticulously review client cases and communicate suitable housing transitions.
- Specialist Support:
Navigating the complexities of the Homelessness Reduction Act and the Care Act to secure the best outcomes for single homeless adults.

Aboutthe Team
You will be joining a dedicated and diverse multi-disciplinary team at the Osborne Grove Assessment Centre, a key component of our wider Rough Sleeping Programme. Our team is committed to providing high-quality support and clear pathways for individuals transitioning away from street homelessness.
As the Move-On Coordinator, you will work at the heart of a supportive internal structure that includes our Service Manager, Deputy Manager, Case Coordinator, and a frontline team of Assessment and Move-On Workers. We pride ourselves on a collaborative culture where information is shared effectively to ensure the best outcomes for our resident.
About You- Experience in coordinating move-on activity from assessment centres or supported accommodation into long-term housing
- Experience working with single homeless adults and people who have been affected by rough sleeping
- The ability to work effectively with Adult Social Care, housing pathway managers, and private rented sector (PRS) partners to unblock barriers for our residents
- Strong organizational skills with a keen eye for detail, particularly when it comes to tracking progress and maintaining high-quality case records
- Exceptional interpersonal skills, with the ability to lead multi-agency meetings and communicate clearly with a wide range of agencies
