Social Worker – Adults Learning Disabilities (Community Care)

Location:

London Borough of Merton

Rate: £32 per hour (Umbrella £24 PAYE)

Contract:

Temporary (with strong extension potential)

Hours:

Monday – Friday, 9:00 – 17:00

Working Pattern:
Hybrid (3 days office / 2 days home)

• Undertake the full Care Management process in line with the Care Act and statutory guidance
• Manage a caseload as allocated by the Team Manager, maintaining accurate and up-to-date records (paper and IT systems)
• Complete risk assessments in line with Merton Care Management Procedures
• Deliver duties under relevant legislation including the Mental Capacity Act and Deprivation of Liberty Safeguards (DoLS)
• Support safeguarding enquiries and promote Making Safeguarding Personal Commission care packages, ensuring value for money and quality service delivery
• Maintain accurate records on Care First and other systems
• Attend meetings, court hearings, and tribunals as required
• Engage in joint working with partner agencies and stakeholders
• Contribute to service development, data reporting, and continuous improvement

• Qualified Social Worker with relevant registration
• Experience working within Adult Social Care, ideally Learning Disabilities
• Knowledge of the Care Act, MCA, DoLS, and safeguarding processes
• Strong assessment, communication, and case management skills
• Ability to work both independently and as part of a team
