Enhanced DBS with Adults & Children's Barred List / Driver Opus people solutions are currently recruiting for an Adult Practitioner for our client based in Telford on a temporary on-going basis until 31 August 2026. About the Role You will support the delivery of statutory Adult Social Care duties in line with the Care Act, Human Rights Act, and Mental Capacity Act.
This role involves working closely with individuals, families, and carers to assess needs, carry out reviews, and develop person-centred support plans that promote independence, wellbeing, and informed choice.
What We're Looking For:
We are looking for a dedicated and compassionate professional committed to delivering high-quality, person-centred support to adults and their families. You will have strong experience in Adult Social Care, particularly in completing reviews and reassessments, alongside the ability to carry out assessments and develop effective, outcome-focused support plans. A strong focus on review work is essential, ensuring care and support arrangements remain appropriate, responsive, and aligned to individuals' evolving needs while promoting independence and wellbeing.
Key Responsibilities Carry out Care Act assessments, with a strong focus on reviews, reassessments, and eligibility decisions Complete Carers Assessments, Mental Capacity Assessments, and contribute to Best Interest decisions with appropriate oversight Develop strengths-based, person-centred support plans that maximise independence Enable access to community services, assistive technology, and digital solutions Promote the use of personal budgets, direct payments, and increased choice and control Identify, assess, and manage risk, supporting positive risk-taking approaches Maintain accurate, up-to-date case records in line with professional standards Contribute to safeguarding activity, audits, and ongoing service improvements Work collaboratively within multi-agency teams and with community partners Support individuals and families to make informed decisions and achieve improved wellbeing outcomes
