Role Overview

A Senior Adult Social Worker is needed for the Preparation for Adulthood Team at Hillingdon Council. The role supports young people transitioning into adult services, working with Initial Assessment, Locality and Hospital Teams, and the specialist Learning Disability Service.

Responsibilities
  • Work collaboratively with the Transition Service, Children with Disabilities Team, Leaving Care Team, and Special Educational Needs team.
  • Conduct timely and responsive assessments, re‑assessments and reviews to provide person‑centred care that promotes independence and safety.
  • Manage cases including Education, Health and Care Plans (EHCPs), health action planning and other person‑centred planning.
  • Participate in Continuing Health Care (CHC) assessments when appropriate.
  • Support young people in their transition to employment.
  • Liaise with statutory, voluntary organisations and other agencies to ensure effective service delivery.
  • Represent the service in team meetings and developments, sharing information and knowledge.
  • Undertake Mental Capacity Act assessments, Best Interest and Community Deprivation of Liberty orders.
  • Handle safeguarding referrals, risk assessments and protection plans.
  • Provide duty and senior support and offer guidance within the wider Learning Disability teams.
Qualifications
  • Qualified Senior Social Worker with statutory duties experience under the Care Act and Safeguarding.
  • Experience delivering care plans, assessments and smooth transitions for young people into adult services.
  • Strong advocacy, communication and relationship‑building skills.
  • Capacity to work independently and as part of multidisciplinary teams.
  • Commitment to continuous learning and innovative practice.
