Manchester Job Type: Full-time Join our dedicated team in the Leaving Care, Children’s & Education Services in Manchester, where you will play a crucial role in supporting young people transitioning to adulthood. This role is ideal for someone passionate about making a significant impact in the lives of vulnerable residents, ensuring they are prepared and supported to live independently. Day-to-day of the role:
Manage a caseload and maintain accurate case records, undertaking statutory recordings. Contribute to Pathway Plan Assessment and provide ongoing assessment to inform Pathway Planning. Undertake assessments for young people resuming education/training or presenting with other support needs post 21 years. Promote welfare and ensure the views and wishes of young people are considered in assessments, Pathway Plans, and reviews. Provide advice, practical assistance, and support to young people to enable successful transitions.

Coordinate the provision of services and ensure young people make use of such services. Identify and supervise the delivery of support packages for young people living in the community. Ensure young people receive appropriate financial support and help them develop financial management skills. Advise and support young people in gaining access to appropriate accommodation. Work in partnership with a range of agencies to ensure an integrated, holistic response to meet the needs of young people.

Required Skills &

Qualifications:

Strong interpersonal skills with the ability to understand and communicate effectively with others. Analytical skills to engage with stakeholders and identify information needs. Excellent communication skills, both written and verbal. Proficient in planning and organising multiple tasks effectively. Problem-solving and decision-making abilities. Competent administrative and IT skills to maintain effective systems. Understanding of relevant legislation and statutory guidance including the Children Act 1989, Children (Leaving Care) Act 2000, and related regulations.

Willingness to undergo a DBS check. Full driving license and access to a vehicle daily. Flexible to work between 08:00 - 20:00, Monday to Friday.
