Would you like to join a dynamic, high‑performing hospital discharge social work team with no waiting‑list cases, strong values, and exceptionally supportive management. In this role, you will contribute to a service operating at the forefront of integrated Health and Social Care, delivering safe, timely, person‑centred outcomes for residents through close partnership with health colleagues and multidisciplinary teams.

As a Senior Social Worker in the Richmond, Response & Rehabilitation (RRRT) Hospital Discharge Team, you will lead complex discharge planning, oversee hub management, champion strengths‑based practice, and drive excellence across Richmond and Wandsworth. You will work within a collaborative MDT environment supported by proactive, accessible management that enables confident decision‑making and continuous professional growth.

This role places you at the centre of our objective to deliver the best for residents—promoting wellbeing, independence, and active participation in community life while ensuring safe, timely hospital discharges.

This is an excellent opportunity for experienced Social Workers seeking progression, and for Senior Social Workers wishing to advance their practice within the future of Adult Health and Social Care. You will bring a strong interest in integrated working with Health colleagues and multidisciplinary teams to support safe, timely hospital discharges.

You will join a high‑performing service committed to delivering the right care, in the right place, at the right time. Working across several hospital sites, you will collaborate closely with community Health and Hospital Trust partners to ensure effective, person‑centred discharge pathways.

Senior Social Workers play a key role in maintaining high‑quality professional practice.

You will need:
  • The use of clear verbal and written communication skills and the ability to represent Adult Social Care confidently in a range of professional forums.
  • To support the management of the team to deliver a person‑centred service that promotes independence and wellbeing for residents leaving hospital.
  • Duty, formal supervision, co‑ordination of referrals, oversight and clear decision making and attending hospital discharge planning meetings. You will be a safeguarding adults manager (SAM) or willing to train as part of this role.
  • Experience of Hospital discharge (or a willingness to gain experience)
  • Ability to complete Care Act and Mental Capacity Assessments
  • Previous experience or keen to learn how to supervise others

You will work a 36‑hour week within a directorate committed to Continuing Professional Development, strengths‑based practice, and forward‑thinking professional standards. You will receive supportive supervision, training, and opportunities to develop your career while contributing to positive outcomes for residents.

Essential Qualifications, Skills and Experience
  • SWE registration- with a recognised Social Work qualification and evidence of ongoing professional development.
  • Supervision skills
    -ability to provide reflective, supportive and accountable supervision to staff.
  • Duty and workflow management - experience managing duty referrals and making confident, proportionate decisions in a fast‑paced hospital discharge setting.
  • Care Act and MCA competence - strong understanding of Care Act duties, Mental Capacity Assessments and best‑interest decision‑making.
  • Integrated MDT working - proven ability to work collaboratively with health partners, therapy teams and community services to deliver safe, timely discharge pathways.
  • High‑quality communication - excellent written and verbal communication with confident, evidence‑based decision‑making.
  • Strengths‑based practice - commitment to promoting independence, wellbeing and person‑centred outcomes.
