Sanctuary Personnel is seeking a dedicated Social Worker for a permanent position in the Hospital Discharge Team based in Wandsworth. The role is full-time with a salary up to £52,627 per annum.
You will work under supervision, manage a caseload, and use a strengths-based, person-centred approach to support residents and carers in achieving outcomes. You will undertake comprehensive assessments, coordinate care plans, and ensure accurate, confidential recording.
