Change Grow Live in Newham is looking for a Young Persons Transitional Worker to support individuals up to age 25 as they transition into adult services. You will deliver assessment, care-planned treatment and harm-reduction support, coordinating transitions into education, employment or training where possible.
You will work within the Young Persons team, applying evidence-based interventions and liaising with health, social care and youth services to ensure robust referral pathways.
