London Borough of Hillingdon is seeking a Brokerage Officer to join our Social Care Brokerage Service. You will source and coordinate care and support services to meet assessed needs, arranging home care, residential and nursing placements, and specialist support across education, health and social care.
Working with social workers, health colleagues, providers and families, you will manage placement requests, negotiate fees, maintain records and report on performance.
