Adults Social Worker - Hospital Team

Our Client, Bury, is seeking an Adults Hospital Social Worker to join their Team. Fantastic payrate of £30 per hour! Are you an Adults Social Worker, passionate about making a difference in your community? Do not miss out on this fantastic opportunity! Responsibilities of this role:
Conduct comprehensive assessments of adult patients' physical, emotional, and social needs to develop a deep understanding of their individual circumstances. Coordinate and facilitate safe and timely discharge plans. Provide advocacy and support to patients and their families. Conduct risk assessments and implement safeguarding measures to ensure the well-being and safety of vulnerable adults during the discharge process. Respond to urgent requests to visit anywhere in the borough.

Requirements of this role:
Must be a Qualified Social Worker, registered with Social Work England. Must have a Social Work qualification (Diploma in social work, social work degree or equivalent) Must have minimum of 3 years experience working as a Qualified social worker in a similar team
