SW Locums is recruiting an experienced Children’s Social Worker for a full-time locum opportunity within the Transitions and Leaving Care Service at the London Borough of Bexley.
Role Details
* £34.00 per hour

* Combination of office, home and community-based working
About the Role
You’ll support young people in care and care leavers as they develop independence skills and prepare for adulthood.
Key responsibilities include:
* Completing assessments and personalised pathway plans
* Supporting access to education, employment, accommodation and health services
* Promoting the voice and aspirations of young people
* Working with housing, education, health and community partners
* Undertaking visits, reviews and statutory visits
* Assessing risk and identifying support needs
* Advocating for young people's rights and wellbeing
* Maintaining accurate case records and documentation
