About the Role

We are offering an exciting opportunity for an experienced practitioner to join our Liaison and Intensive Support Team (LIST). You will work with children and young people presenting with moderate to severe and complex mental health difficulties, delivering intensive interventions within community and acute settings.

In this role, you will:

  • Deliver high-quality, timely mental health assessments for children and young people in crisis.
  • Provide short-term, intensive interventions to stabilise mental health and reduce risk.
  • Develop and implement evidence-based, person-centred care plans.
  • Work flexibly across community, home, and acute environments, including liaison with acute hospitals where required.
  • Collaborate closely with the multidisciplinary team, including Consultant Psychiatrists, psychologists, nurses, OT and wider services.
  • Support safe and effective transitions between services, including inpatient care and community pathways.
  • Coordinate care and work proactively with partner agencies (e.g. social care, education, paediatrics) to reduce escalation and prevent unnecessary admissions.

Your clinical judgement, ability to work under pressure, and commitment to best practice will be central to achieving positive outcomes for young people and their families.
