A key aspect of the role involves undertaking Care Act assessments and creating care plans that focus on individuals' abilities and promote independence where safe and proportionate.

About

You

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of 2 years' experience is essential to be considered for this role. Experience with in a mental health team lends well to the success of this position.
