We are looking for dedicated and skilled Family Support Practitioners to join our Early Help service and play a key role within our Family Hubs. This is an exciting opportunity to work alongside families, Family Hub Navigators, schools, health professionals, social care colleagues and community organisations to provide integrated support that helps families thrive.

About the Role
As a Family Support Practitioner
, you will work directly with children, young people and families who are experiencing challenges that may impact their wellbeing, relationships and future outcomes. You will hold a caseload and provide targeted, strengths-based interventions that help families build resilience, achieve positive change and access the right support at the right time.

What You'll Do
You will:

• Provide direct support to children, young people and families facing a range of challenges including financial hardship, housing difficulties, parental mental health, SEND, domestic abuse, family breakdown and education concerns.
• Complete assessments, develop outcome-focused support plans and monitor progress with families.
• Work collaboratively with Family Hub Navigators and partner agencies to ensure families can access universal, targeted and specialist support services.
• Champion the voice of children and families, ensuring their views shape planning and decision-making.
• Contribute to multi-agency working, including Team Around the Child, Early Help and other partnership meetings.
• Deliver support that is family-centred, strengths-based and focused on sustainable outcomes.
• Maintain accurate records and evidence impact through excellent case management and reporting.

About Family Hubs
Family Hubs are welcoming, inclusive community spaces where families can access information, advice and support from pregnancy through childhood and adolescence. They bring together services across health, education, SEND, social care and the voluntary sector to create a "one-stop-shop" approach for families.

As a Family Support Practitioner, you will be a vital part of this model, helping families move smoothly between universal services and targeted support, ensuring no family has to navigate challenges alone.

About You
We are looking for someone who:

• Has experience of working with children, young people and families in an early help, education, health, social care or related setting.
• Is committed to relationship-based and strengths-focused practice
• Has excellent communication, organisational and partnership-working skills.
• Can manage a varied caseload and work effectively within a multi-agency environment.
• Understands safeguarding responsibilities and is committed to achieving positive outcomes for children and families.
• Is passionate about reducing inequalities and supporting families to achieve their full potential.
