Location: Greater London
Single Homeless Project hires a Homeless Pathway Coordinator to support patients experiencing homelessness. The role involves working on wards and A&E, assessing needs, and ensuring voices remain central in decisions about health, housing and support systems.
You will manage caseloads, develop clear discharge and support plans, and refer to housing, health and social services while advocating for safe accommodation and appropriate support.
