Join our dedicated Hospital Team in Newham as a Qualified Social Worker. In this role, you will jointly work with healthcare professionals to assess, plan, and implement effective interventions, ensuring that our clients receive the highest standard of care and support during their hospital stay and upon discharge.
- Conducting comprehensive assessments to identify the needs and risks of adults requiring social work intervention.
- Developing and implementing care plans in collaboration with multidisciplinary teams and service users.
- Providing emotional support and practical assistance to clients and their families.
- Ensuring compliance with relevant legislation and safeguarding policies.
- Maintaining accurate and up-to-date case records and documentation.
- Proven experience in adult social work, particularly within a hospital or healthcare setting.
- Strong assessment and analytical skills with the ability to manage complex cases.
- Excellent communication and interpersonal skills, with a compassionate approach to working with vulnerable adults.
