Local Authority Social Work, Care Management / Care Assessor, East London Pay rate £27.88 per hour Contract role, Care Management Vitalis are recruiting for experienced Care Manager for a Local Authority in East London This is to work in the Havering Access Team This is work within Care Management, and you will need experience in a Local Authority Social Care team.

This role may also suit a Newly Qualified Social Worker with experience within Adult Services. Complete high-quality, holistic Care Act assessments, reviews, carers assessments and support planning using strengths-based and person-centred practice. Manage enquiries and referrals received by telephone, email, electronic referral routes and face-to-face contact. Gather, analyse and record relevant information, provide clear advice and identify the most appropriate next steps.

Assess and manage risk with adults and carers, escalating safeguarding or complex matters in line with Council procedures. Apply the principles of the Care Act 2014, Mental Capacity Act, safeguarding, data protection and other relevant legislation in day-to-day practice. Work collaboratively with social workers, occupational therapists, health partners, community services and the person’s own support networks. Promote independence, prevention and early intervention, including the use of community resources, equipment, assistive technology and personal budgets where appropriate.

Maintain timely, accurate and evidence-based records on Adult Social Care systems and contribute to service standards and performance requirements. Plan and prioritise an appropriate caseload, seek support when needed and keep relevant colleagues informed of progress or concerns. Undertake community visits as required and contribute positively to duty arrangements and wider team activity.
