Leaving Care Support Worker

We are seeking a dedicated Leaving Care Support Worker to join our Children’s Services team. This role is crucial in assisting young people aged 16-25 transitioning to adulthood, particularly those leaving care or facing homelessness. The successful candidate will work within a supportive team environment to help these young individuals build their futures through tailored support and guidance. Day-to-day of the role:
Act as a Personal Advisor to young people, providing crisis and long-term support. Manage individual caseloads, liaising with Pathway Coordinators, social workers, and other service providers. Prepare, produce, implement, and evaluate plans with individuals, families, and professional colleagues. Assess needs and recommend courses of action to support young people in achieving positive outcomes. Contribute to the planning and development of team and departmental services.

Maintain accurate records and participate in decision-making forums. Work collaboratively with partner agencies to provide comprehensive support services. Required Skills &

Qualifications:

Knowledge of issues facing young people leaving care or becoming homeless. Understanding of relevant childcare legislation and policy. Proficient in counselling, welfare rights, and IT support skills. Excellent communication skills, both verbal and written. Ability to undertake assessments and review programmes, including pathway plans. Demonstrated organisational skills with the ability to meet deadlines. NVQ Level 3 Caring for Children and Young People or equivalent qualification.

Experience working with young people in a similar setting is highly desirable. Benefits:
