Are you a qualified Social Worker with experience supporting adults with mental health needs ? We are currently looking for a compassionate and experienced Mental Health Social Worker to join a dedicated team and support individuals to achieve greater independence, stability and wellbeing.

The Role As a Mental Health Social Worker, you will work with adults experiencing a range of mental health needs, providing person-centred support and helping individuals access the appropriate services and care. Your responsibilities will include:
Completing assessments and reviews of individuals' care and support needs. Developing person-centred care and support plans. Supporting individuals to achieve positive outcomes and maintain their independence. Working with individuals, families and carers to understand their needs and identify appropriate support. Managing a varied and complex caseload. Working closely with mental health professionals and multidisciplinary teams. Liaising with health services, housing providers, voluntary organisations and other agencies.

Identifying and responding appropriately to safeguarding concerns. Supporting individuals through transitions and changes in their care. Maintaining accurate and up-to-date records and case notes. Working in accordance with relevant legislation, policies and safeguarding procedures. About You We are looking for a Social Worker who:
Holds a recognised Social Work qualification . Is registered with Social Work England . Has experience working with adults experiencing mental health difficulties. Has experience completing assessments, reviews and care planning. Has a good understanding of safeguarding and relevant social care legislation. Is confident managing a complex caseload. Has excellent communication and interpersonal skills. Can work effectively as part of a multidisciplinary team.
