About the Role
Newham Council is seeking an experienced and motivated Adult Hospital Practice Manager to join its Hospital Social Work Team.
This is an excellent opportunity to lead and support frontline social work professionals, working collaboratively with acute hospital teams and multi-disciplinary colleagues to facilitate safe, timely and effective hospital discharges.
The successful candidate will play a key role in delivering person-centred assessments, managing complex cases and ensuring adults receive the appropriate care and support to promote independence, wellbeing and positive outcomes.
Key Responsibilities
* Manage, supervise and support frontline social work staff within the Hospital Social Work Team.
* Work closely with hospital and health professionals to facilitate timely and safe hospital discharges.
* Oversee Care Act assessments, Carers Assessments, enablement referrals and person-centred support planning.
* Ensure high-quality assessments, analytical risk assessments and effective care management.
* Support strength-based approaches that promote independence and meet individual needs.
* Manage complex cases involving older adults, physical disabilities, sensory loss, learning disabilities and dementia.
* Ensure practice complies with the Care Act 2014 and relevant adult safeguarding procedures, including the Pan-London Adult Safeguarding framework.
Essential Requirements
* Recognised Social Work qualification.
* Current Social Work England (SWE) registration.
* Relevant experience in Adult Social Care.
* Strong understanding of assessment, care management and the Care Act 2014.
* Knowledge of Pan-London Adult Safeguarding procedures.
* Experience in staff supervision, practice management and multi-disciplinary working.
