Case Manager

You’ll work for Northeast Family Services, a mental health provider with over 20 years of experience serving children, youth, and families. The case manager reports to the Clinical Supervisor and collaborates with the clinical team to serve as a liaison among the treatment team, clients, families, and collateral service providers.

• Maintain assigned caseload and ensure proper implementation of in‑home services in accordance with referrals made by DCYF.
• Respond to referral requests, create treatment plans, participate in treatment meetings, and provide regular service‑specific documentation.
• Collaborate on treatment plan formation with measurable and attainable goals in partnership with families and DCYF.
• Adhere to all documentation requirements of service delivery including assessment and report writing.
• Report progress to the court system through in‑person appearances and written reports as requested.
• Provide parent education and supervised visitation to children, adults, and families in home and community‑based settings.
• Offer parent coaching and support including effective parenting strategies, meeting healthcare and educational needs of children, understanding developmental milestones, meal preparation, and budgeting.
• Enforce supervised visitation expectations identified by DCYF to ensure child safety.
• Interface with DCYF, the NH court system, schools, behavioral health providers, and other members of the treatment team as necessary.
• Assess client risk on an ongoing basis and consult with the clinical supervisor regarding safety concerns.
• Provide transportation to clients, including installation of car seat and booster seat.
• Participate in an on‑call rotation, responding to phone calls to provide support to families as needed.
• Work early evenings and have flexible availability to meet client needs.
• Document all clinical contacts and activities through the Electronic Health System.
• Perform other duties as assigned.

• Bachelor’s degree in social work, counseling, psychology, or a related human services field.
• Minimum of two years of experience working with children, youth, and families in a home or community setting (preferred).
• Valid driver’s license and reliable access to a vehicle.
