At our rapidly growing practice, Cardiovascular Specialists of New England, we are seeking a highly motivated Medical Assistant Care Coordinator. In this role you will work directly with an interdisciplinary team of healthcare professionals to facilitate outstanding care for our patients. You will manage and coordinate patient care following admissions, procedures, and discharges, ensuring safe transitions, early identification of complications, follow-up appointments, adherence to treatment plans, and serving as the central point of communication among the practice, cardiologists, primary care providers, and patients.

You will report directly to the Clinical Nurse Manager.

• Receive and review procedural notes for cardiology patients (PCI, pacemaker/ICD implantation, ablations, heart failure admissions, valve procedures).
• Contact patients within the defined timeframe (typically 24–72 hours post-discharge) to conduct medication reconciliation, verify prescriptions, anticoagulation plans, and medication changes, execute post-discharge plans as per orders, ensure patients understand discharge instructions, and elevate clinical needs to the care team.
• Schedule and coordinate follow-up visits with cardiologists, advanced practice providers, and diagnostic services (echocardiograms, stress tests).
• Collaborate with the Intensive Cardiac Rehabilitation team to facilitate enrollment of appropriate patients or referral to traditional cardiac rehab when necessary.
• Support adherence to care plans by addressing barriers such as transportation, medication access, or understanding of instructions.
• Serve as a clinical resource to patients and families, offering guidance and triage as appropriate.
• Document patient interactions thoroughly within the electronic medical record (EMR) and raise issues to the care team as needed.
• Assist with remote patient monitoring programs (heart failure weight checks, vital sign monitoring, device clinic communication).

• Collaborate with cardiologists, APPs, nurses, and hospital transition-of-care teams to ensure continuity.
• Provide updates on high-risk patients and participate in interdisciplinary meetings or huddles.
• Act as a liaison between the cardiology practice and external providers, including rehabilitation programs, skilled nursing facilities, and home health agencies.

• Support quality improvement initiatives related to readmission reduction, patient satisfaction, and care transition workflows.
• Ensure compliance with all regulatory and documentation requirements.

• Certified Medical Assistant
• Current BLS certification
• Minimum 2 years of clinical cardiology experience
• Minimum 2 years of transitional care management and/or care coordination of cardiology patients
• Knowledge of cardiovascular disease, diagnoses, and medications
• Ability to interpret clinical documentation and cardiac terminology
• Strong organizational skills with attention to detail
• Proficiency with Athena EMR systems and care-management platforms
• Compassionate, patient-centered mindset

You will work out of the Londonderry, New Hampshire office.

Full-time, hourly range of $23–$27 per hour plus medical, dental, and vision benefits. We offer a matching 401(k), 12 days paid time off, and 8 paid federal holidays.
