Position SummaryWe are seeking compassionate, bilingual, and mission-driven Clinical Nurse Practitioners (NPs) to support our network of Primary Care Providers (PCPs) operating under Full Risk Medicare Advantage Value-Based Care (VBC) Contracts. These NPs will be embedded within IPA member practices to deliver Annual Wellness Visits (AWVs), disease screenings, chronic care assessments, and support practices in closing care gaps related to HEDIS, STARS, and Accurate/Compliant Documentation performance.
Ideal candidates are bilingual in Korean and English, possess a strong sense of empathy and care for patients, and are eager to learn.
Comprehensive VBC-focused training will be provided to ensure success in the role.
Key Responsibilities- Perform Annual Wellness Visits (AWVs), preventive screenings, and chronic care management in collaboration with IPA-affiliated primary care physicians.
- Conduct disease detection screenings such as diabetic eye exams, colorectal screenings, fall risk, and cognitive impairment assessments.
- Address and close clinical care gaps tied to HEDIS, STARS, and quality measures.
- Deliver empathetic, patient-centered care aligned with VBC objectives and promote patient trust and engagement in preventive and chronic care.
- Educate patients on their conditions and treatment plans using culturally and linguistically appropriate communication methods.
- Collaborate with MSO care management, quality, and risk adjustment teams to elevate complex cases and support wraparound care.
- Accurately and compliantly document diagnoses, assessments, and care plans in accordance with VBC clinical documentation guidelines (e.g., MEAT criteria, CMS HCC coding standards).
- Collaborate with the MSO’s Risk Adjustment and CDI teams to optimize visit workflows and support documentation compliance/accuracy.
- Apply best practices for chronic condition management and diagnosis coding to support accurate/ compliant documentation.
- Participate in chart reviews, risk audits, and continuous improvement initiatives to support VBC performance targets.
- Serve as an onsite clinical resource to practice staff during transition to or performance improvement in VBC models.
- Support adoption of MSO tools such as pre-visit planning checklists, coding templates, clinical dashboards.
- Provide field feedback to MSO leadership to improve support programs and operational alignment.
- Education & Licensure: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) required.
- Board-certified Nurse Practitioner (FNP, AGNP, or equivalent).
- Active and unrestricted California NP license.
- DEA License and current NPI required.
- BLS Certification.
- Experience: 1 year of experience in primary care, geriatrics, or chronic disease management preferred; experience in Medicare Advantage, ACOs, or other VBC models highly desirable; newer graduates encouraged.
- Must demonstrate fluency in Korean and English, both written and verbal.

- Work Environment & Travel: Must be able to travel to assigned practice locations within LA/Koreatown or Orange County.
