Job Overview:
The Social Worker facilitates treatment and provides coordination of referrals and intakes, direct practice in the areas of group work, consumer education, treatment planning and individual counseling. Participate as a member of the multidisciplinary treatment team, performing intakes, assessments, initiating treatment interventions and crisis intervention. Perform clinical case management and treatment planning with consumers that experience psychiatric disorders and substance abuse disorders.
All clinical documentation is completed with compliance in the requirements set by the NJ Division of Mental Health Services and the Joint Commission on Accreditation of Health Care Organizations. Participates in staff training and continuing education. Takes an active role in Performance Improvement projects. Is responsible for reporting identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors, etc.
Qualifications:
* Master's Degree in Social Work or related mental health service discipline
* Current NJ LSW or LAC Licensure
* Experience providing both individual and group therapy, intake assessments, treatment planning and crisis intervention

Essential Functions:
* Assume active participation in Performance Improvement Projects.
* Initiate improvement efforts and work collaboratively with the treatment team to identify areas of needed improvement.
* Attend In-service training to improve skill level and knowledge base. Participate in training preparation and planning process when required.
* Support the Department's in-service and continuing education committee.
* Attends and participates in multidisciplinary staff meetings and is prepared to discuss clinical and program issues. Complete documentation in the medical record according to department policy. Psychosocial Assessments, Group Notes, Progress Notes and treatment summaries include all required information according to policy.
* Coordination of referrals and intakes from PESS, IP, PHP, State Psychiatric Hospitals, Community and other hospital systems including but not limited to scheduling and keeping track of the status of these referrals. Keeping statistics on the outcomes of these referrals.
* Facilitate Patient Education through individual and group counseling. Prepare education materials for distribution and presentation to mental health consumers. Facilitate Inpatient and Outpatient Patient and Family Psycho-education. Provide information and support for patients and family members prior to and after Inpatient Discharge.
* Facilitate Intake Coordination through management of intake line referrals, coordination of arriving intakes, work to assure full schedule for all Social Work Intake Assessment Providers, schedule Community Intakes and Inpatient Intakes, request med appointments.
* Facilitate referrals to other service agencies and assist clients in obtaining appropriate entitlements (Social Security Benefits, Residential program, Self Help).
* Function as liaison with the inpatient unit, specifically completing assigned consults within 48 hours and attending alternate treatment team meetings with the inpatient.
* Organize work and set priorities with a minimum of supervision and seek guidance as appropriate.
* Make decisions that are timely and consistent with program objectives, policies and procedures.
* Participate in the departments QA/PI efforts. Complete QA/PI monitors and follow-up on action plans.
