OverviewVNA Health Group is seeking a compassionate Licensed Social Worker (LSW) to join our Hospice Team. The Hospice Liaison plays a key role in assessing patients for hospice eligibility, coordinating smooth transitions of care, and providing emotional and psychosocial support to patients and families.

Responsibilities- Conduct hospice assessments and admissions in home and facility settings.
- Educate patients, families, and community partners about hospice philosophy and services.
- Provide emotional support and counseling during the referral and admission process.
- Collaborate with physicians, discharge planners, and the interdisciplinary team to ensure continuity of care.
- Participate in outreach and education to increase awareness of hospice services.
- Master’s Degree in Social Work (MSW) required.
- Active New Jersey LSW license required.
- Minimum 2 years of experience in healthcare, hospice, or homecare.
- Strong communication, counseling, and organizational skills.
- Valid NJ driver’s license, vehicle, and proof of insurance.
- Over 113 years of trusted home and community-based care.

CompensationUSD $60,000 to USD $110,000.
