. We have an exciting opportunity for a Registered Dietitian to coordinate nutritional care for long term and sub-acute care patients.
- Conducting nutrition assessments
- Participating in care plan meetings
- Providing education to patients and families
- Obtaining food preferences
- Providing support.
Qualified candidates will have a Bachelor's degree and familiarity with DOH regulations/standards, the state survey process, electronic medical records, Microsoft Office, MDS 3.0, knowledge of Geri-menu.
