Sunday - Thursdays 8:30am - 5:00pm
About the Role: The Patient Care Concierge is a Certified Nursing Assistant role dedicated to enhancing the overall experience of patients and families in the short-term rehabilitation unit. This position supports both the nursing and life enrichment departments by providing proactive customer service, additional CNA assistance, and meaningful engagement with patients. The Patient Care Concierge serves as a liaison between patients, families, and staff to promote satisfaction, communication, and high-quality resident-centered care.
Key ResponsibilitiesNursing and CNA Support:
- Proactively rounds on short-term rehab patients to assess comfort, needs, and satisfaction
- Serves as a friendly and consistent point of contact for patients and families
- Addresses minor concerns in real time and escalates concerns as appropriate
- Assists with new patient orientation and supports the discharge experience
- Assists CNAs during peak times with call lights, toileting, transfers, and ADLs
- Provides coverage during CNA breaks as needed
- Supports nursing staff with non-licensed duties within CNA scope of practice
- Assists with group and one-on-one activities for rehab patients
- Encourages patient participation in activities and engagement opportunities
- Transports patients to activities as needed
- Provides individualized engagement for patients unable to attend group activities
- Supports special events, family visits, and rehab-focused programming
- Communicates patient preferences, concerns, and feedback to the interdisciplinary team
- Documents interactions per facility policy
- Escalates customer service issues promptly and appropriately
- Collaborates with nursing leadership and life enrichment staff to improve patient experience
