This full-time role involves collaborating with health professionals to facilitate patient transitions to Long Term Care homes. You will utilize your nursing expertise to reduce hospital readmissions and enhance clinical practices within your team. Key Responsibilities:
• Provide clinical assessments and treatments in LTC
• Collaborate on discharge strategies to reduce hospital visits
• Train staff using best practice guidelines
• Evaluate care programs for continuous improvement
• Ensure effective communication across care facilities
Requirements:
• Master’s in Nursing/Nurse Practitioner or equivalent
• Current CNO registration as an NP
• 1-3 years of practical experience as a Nurse Practitioner
• Previous experience in Geriatrics preferred
• Strong leadership and communication skills
