Nurse Practitioner at Southwest Integrated Health

This full-time, permanent role reports to the Primary Health Care Manager. You will engage in health promotion, preventive services, and emergency care, ensuring the health needs of the community are met. Your efforts will focus on holistic care approaches, especially for at-risk youths and families.

Key Responsibilities:

• Deliver primary health care and emergency services

• Assess patient health status and provide recommendations

• Facilitate and implement community health initiatives

• Conduct laboratory and imaging test interpretations

• Collaborate with various health partners to improve services

Requirements:

• Master of Nursing - Nurse Practitioner (MN-NP)

• Community development and health promotion experience

• Expertise in therapeutic interventions and case management

• Skills in evaluating and addressing health determinants

• Commitment to supporting youth prenatal care
