Job Scope
Under the direct supervision of LPHA and/or Peer Specialist Supervisor (PSS), the Care Coordination Peer Specialist draws from their own personal lived experience to provide individualized support to Community Health Core individuals working on their recovery goals. The Peer Support Specialist shares hope, and exercises Person Centered/Trauma Inform services to support that individual to live, work and participate in their home and community.
The Specialist will provide resources including housing, employment and access to resources to meet basic needs, primary care, and linkage to natural supports. Services are typically provided in the individual's home, Community Health Core Office, or community setting. The Peer Support Specialist must be able to establish good personal boundaries and communicate effectively.
High school diploma or GED. Must have a valid driver's license. Upon hire, successful completion of training and certification for Mental Health Peer Specialist is required to meet Health and Human Services Commission (HHSC/Medicaid) guidelines.
Experience and KnowledgeRequires advanced organizational skills, ability to manage multiple tasks, typing and computer skills. Must demonstrate understanding of culturally sensitive, trauma informed, and person centered principles and practices. Requires understanding of mental illness, behavioral health issues, assessment processes and how to access professional/community resources. Must have excellent written and verbal communication skills.
Physical RequirementsMust be able to adequately complete SAMA, First Aid and CPR training. Work endurance sufficient to work an 8-12 hour day, mobile and able to bend and reach during the 8-12 hour day.
Minimum Hourly Salary$16.00 per hour
