Job SummaryWe are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team as a Field LVN in Longview, TX. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their health and comfort while promoting independence and well-being.
Key Responsibilities- Provide skilled nursing care to patients in their homes according to physician orders and care plans.
- Monitor patient health status and report any changes or concerns to the supervising nurse or physician.
- Administer medications, injections, and treatments as prescribed.
- Assist patients with activities of daily living (ADLs) as needed.
- Document patient care accurately and timely in compliance with agency policies and regulatory requirements.
- Educate patients and their families on health management, medication administration, and disease prevention.
- Collaborate with healthcare team members to coordinate patient care and ensure continuity.
- Maintain patient confidentiality and adhere to HIPAA regulations.
- Current and valid LVN license in the state of Texas.
- Minimum of 1 year nursing experience preferred, preferably in home health or related field.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively in a field setting.
- Reliable transportation and valid driver’s license.
- CPR certification preferred.
This position involves traveling to patient homes within the Longview, TX area. The Field LVN will work independently in a variety of home settings and must be adaptable to different environments and patient needs.
