RN Hospice House – Richard Nau Hospice House – Peace Health

Peace Health is seeking a RN Hospice House – Richard Nau Hospice House for a per‑diem/relief, 0.00 FTE, variable position. The salary range is $50.14–$78.34 per hour, depending on education, training, work experience, collective bargaining agreement terms, seniority, and other factors.

Job Summary

This position is responsible for providing direct skilled nursing care and coordination of patients admitted to hospice house inpatient services, making patient‑care decisions using professional healthcare processes of assessment, problem identification, planning, intervention, and outcome evaluation.

Essential Functions
  • Perform initial evaluation of patient and apply admission criteria to determine if the patient is a safe candidate for home health/hospice services.
  • Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the hospice plan of treatment.
  • Performs physical assessments, develops, coordinates, modifies, and manages written individualized plans of care as ordered by the primary physician.
  • Develops and clinically manages the Home Health Aide’s care plan according to the patient’s evolving personal needs.
  • Coordinate care, collaborate, and communicate with members of the interdisciplinary team to ensure appropriate and smooth flow of care through the continuum.
  • Performs discharge planning in line with patient goals and evaluates interventions and outcomes by reviewing established goals.
  • Provide accurate, complete, and timely clinical documentation in compliance with regulatory, quality, and agency guidelines.
  • Assist with the plan and implementation of family bereavement services following patient death.
  • Perform other duties as assigned.
Qualifications Education
  • Accredited School of Nursing – required
  • Bachelor of Science in Nursing – preferred
Experience
  • Minimum of 1 year home health or hospice/end‑of‑life nursing experience – required
  • Hospice experience – preferred
  • Demonstrated knowledge of and ability to apply age‑specific principles of growth and development and life stages to meet each patient’s needs – required
Credentials
  • Registered Nurse in state of practice – required upon hire
  • Basic Life Support – required upon hire
  • Food Handlers Card – Washington – required upon hire
Skills
  • Basic computer skills including MS Office applications – required
  • Well organized, flexible, and able to function independently – required
  • Cooperative working skills, leadership skills, interpersonal skills, and ability to work effectively with patients, families, and staff – required
Working Conditions

Lifting

  • Patient handling no greater than 35 lbs without assistive equipment (NIOSH)
  • Fine motor skills to grasp and control equipment and perform precise procedures
  • Push/Pull up to 45 lbs force (e.g., medical carts)
  • Perform hands‑on CPR at least 20 min using 100–125 lbs of force – seldom or occasionally
  • Ability to move around area with frequent sitting
  • Bending, stooping, squatting, reaching, kneeling frequently
  • Lifting non‑patient up to 20 lbs occasionally and 5 lbs frequently

Environmental Conditions

  • Exposure to biohazard, body fluids, and airborne particles
  • Must be able to complete tasks in a noisy environment

Mental/Visual

  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted)
  • Ability to communicate and exchange accurate information
