Home Health RN Job Details

  Lorain, Ohio (serving Lorain County and surrounding areas)
• Assignment Type:
  Locum tenens, travel nursing contract
• Duration: 13 weeks
• Shifts:
  
  Monday – Friday, 8:00 AM – 5:00 PM
• Shift Duration: 8-hour days
• Hours per Week: 40
• Estimated Weekly Pay: $1,733 – $1,926
• On-Call:

Registered Nurse Qualifications

• Minimum 1 year of recent Home Health RN experience (within the past 3 years)
• Proficiency with HCHB (Homecare Homebase) electronic documentation
• Experience with OASIS documentation
• Active Ohio or Compact Registered Nurse License (must be in hand)
• Current BLS certification

Home Health Nurse Responsibilities

• Provide skilled nursing care to home health patients in Lorain County and surrounding Ohio areas
• Perform comprehensive patient assessments and create individualized care plans
• Accurately document patient care using HCHB and OASIS systems
• Collaborate with interdisciplinary healthcare teams for optimal patient outcomes
• Educate patients and families on disease management, medication administration, and home safety
• Participate in on-call rotation as required
