Time is split 80% and 20%, respectively. Transition Services are provided through a cooperative program Transition Partnership Program (TPP) contract. This contract is between the Department of Rehabilitation (DOR) and Green Dot Public Schools. Case-carriers, school counselors, school psychologists, and school administrators refer students between the ages of 16-21 years old to TPP. The goal of this position is to prepare students with disabilities for employment through DOR Student Services, which includes job exploration counseling, workplace readiness training, work-based learning opportunities, post-secondary education counseling and instruction in self-advocacy.
Support for a larger population of students with disabilities is provided under Green Dot’s Special Education Program in collaboration with case-carriers and school counselors.
ESSENTIAL RESPONSIBILITIES DOR Student Services Program Duties: Educate case-carriers on DOR Student Services and eligibility. Assist potentially eligible TPP students with gathering appropriate documentation for DOR. Identify current TPP student connections to other agencies:
Regional Center, SSI, SSDI. Develop lesson plans and activities for job exploration counseling, workplace readiness training, instruction in self-advocacy and post-secondary education counseling. Identify, assess, and evaluate potentially eligible student’s interests, performance and needs. Provide job exploration counseling. Provide workplace readiness training. Connect students to work-based learning experiences based on strengths and interests. Coordinate and conduct work-based learning experiences, site logistics meetings with business partners, TPP Job Placement Specialist/Job Coach, and TPP students.
Identify and ‘troubleshoot’ problems that occur at work-based learning experience sites with TPP student, TPP Job Placement Specialist/Job Coach, students, employers and case-carriers, such as production problems, the method of performing tasks, relationships with co-workers, boredom, frustration, etc. Provide instruction in self-advocacy. Provide post-secondary education counseling. Collaborate with school counselors to plan and prepare students with self-advocacy skills to leverage post-secondary education and employment supports.
Initiates and maintains contact with a variety of businesses and industry representatives as part of work-based learning experiences. Compile notes for TPP student’s monthly and quarterly progress report based on interactions and supports provided under DOR Student Services. Assist in linking TPP students with public support agencies to help them increase their independence and make progress in DOR Student Services.
Participate in TPP/DOR monthly collaborative and quarterly meetings. Utilize community, post-secondary education and business resources to enhance delivery of DOR Student Services. Participate in community employer networks and Chamber of Commerce meetings.
Special Education Department Duties: Conduct age-appropriate transition assessments based on identified post-secondary outcome goals. Collaborate with students, staff and families in developing annual transition services/activities and IEP goals reflective of the transition assessment(s).
