Position Summary

MAJOR RESPONSIBILITIES/ESSENTIAL FUNCTIONS

The Navigator for Orthopedic and Cardiac Services at Hollywood Presbyterian Medical Center (HPMC) plays a vital role in supporting the hospital's mission to deliver patient-centered, culturally sensitive, and efficient care. This role ensures personalized guidance through the healthcare continuum, emphasizing education, care coordination, and advocacy for patients undergoing orthopedic and cardiac procedures. The Navigator acts as a liaison between patients, families, and healthcare providers to streamline processes and ensure high-quality clinical outcomes and enhance operational efficiency.

Duties

  • Operational Management:
  • Provide tailored education to patients and families regarding orthopedic and cardiac procedures, recovery expectations, and self-care.
  • Coordinate multidisciplinary care plans involving surgeons, cardiologists, case managers, nurses, physical therapists, and social workers.
  • Ensure timely scheduling of surgeries, diagnostic imaging, consultations, and rehabilitation services.
  • Communicate effectively with all stakeholders to enhance continuity and quality of care throughout the patient journey.
  • Monitor and report quality metrics related to patient satisfaction, readmissions, and length of stay for orthopedic and cardiac services.
  • Assist in implementing evidence-based practices and clinical pathways that align with national standards and organizational goals.
  • Support community outreach efforts and provide education to raise awareness of HPMC’s orthopedic and cardiac programs.
  • Work collaboratively with the performance improvement team to achieve accreditation standards and improve patient outcomes.
  • Staff Management:
  • Conduct education for staff on the patient types and case studies with ongoing coaching.
  • Foster a collaborative and supportive work environment.
  • Patient Care:
  • Ensure that patients receive high-quality care in accordance with their treatment plans.
  • Monitor patient progress and adjust care strategies as necessary.
  • Address patient and family concerns with empathy and professionalism.
  • Regulatory Compliance:
  • Ensure compliance with all local, state, and federal regulations and standards.
  • Prepare for and participate in inspections and audits.
  • Maintain up-to-date knowledge of industry regulations and best practices.
  • Review and Develop a plan of correction for deficiencies noted during survey inspections and provide a written copy of such in collaboration with the organization’s regulatory oversite to the governing board and ombudsman representative as required.
  • Financial Management:
  • Monitor revenue and expenses to ensure financial sustainability.
  • Implement cost-control measures while maintaining quality care.
  • Quality Improvement:
  • Develop and monitor quality improvement initiatives.
  • Analyze data to identify areas for improvement in patient care and operational efficiency.
  • Lead staff training on quality improvement practices.
  • Participate in Infection Prevention practices.
  • Committee Functions:
  • Serve on various committees of the facility and provide written/oral reports of such committee meetings as directed or as may become necessary
