Care Manager RN Evening

Overview

Case Management is a collaborative practice that includes patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners, and the community. The process facilitates communication and care coordination along a continuum through effective transitional care management, while recognizing the patient’s right to self‑determination and addressing social determinants of health and complexities of care such as barriers to coping, abuse, mental health, substance use, lack of resources, and psychosocial issues.

• Facilitate communication and care coordination across the continuum of care.
• Provide effective transitional care management and ensure smooth transitions between care settings.
• Recognize and respect patient self‑determination.
• Address social determinants of health and complexity issues including barriers, abuse, mental health, substance use, resource limitations, and psychosocial issues.

• Bachelor’s Degree.
• Upon hire:
  California Registered Nurse License.
• California Fire and Life Safety Card from a National Organization within 30 days of hire.
• Two years of experience in a healthcare‑related field (acute, ambulatory, or post‑acute).
• Two years of experience in acute care management (care coordination or utilization management) or successful completion of the Transitions in Practice (TIP) program for Care Managers.

• Master’s Degree in a health‑related or science field.
