Bilingual Spanish Health Coach and Educator (Home Visits LA/OC, CA w/ Mileage Reimbursement)

Alignment Health is seeking an organized, compassionate, and motivated bilingual Spanish Health Coach and Educator to join the Care Anywhere team in Los Angeles / Orange County, California. As a Health Coach & Educator you will conduct home and virtual visits to develop a wellness coaching relationship with patients, caregivers, and families.

• Must conduct 6-8 home visits per day.
• Mileage reimbursement provided from leaving home to returning.

Schedule:

Monday - Friday, 8:00 AM - 5:00 PM Pacific Time

• Contact patients eligible for Alignment’s Care at Home program and provide wellness coaching to reduce or eliminate high-risk behaviors.
• Collaborate with advanced practice clinicians and/or social worker to develop an action plan.
• Encourage adoption of habits conducive to a higher quality of life.
• Deliver a high level of patient service and increase member retention and satisfaction.
• Assist patients with accessing community resources and social services to address social determinants impacting health and wellbeing.
• Develop and distribute health education materials to identified patients, caregivers, and/or families.
• Coordinate Care with other Alignment team members, including health-related vendors for optimal patient care.

• Minimum 1+ year of experience as a wellness or health coach or educator.
• Preferred:
  Senior care experience.

• Required:
  
  High School Diploma or GED;
  Associate’s degree or two years additional experience in lieu of education.
• Preferred:
  Bachelor’s Degree.

• Strong communication, leadership, and collaboration skills.
• Solid knowledge of diseases, lifestyle topics, nutrition, and exercise.
• Experience applying behavior change models.
• Effective interpersonal skills and trust building.
• Ability to work independently and remotely.
• HIPAA knowledge and confidentiality.
• Proficient in Microsoft Office (Excel, Word, PowerPoint).
• Flexible schedule and problem solving.
• Bilingual English and Spanish (Preferred).

• Current and valid California Driver’s License and reliable transportation.

Pay Range: $48,373.00 - $72,559.00
