Adult Day Program Coordinator

Momentum is currently hiring an Adult Day Program Coordinator to join our team. The role combines transportation duties with direct care at the Day Program in Thousand Oaks, located in the San Fernando Valley.

Base pay range: $20.00/hr – $24.00/hr schedule: 8:00 AM to 4:30 PM, Monday through Friday.

• Ensures safe operation of residential transportation vehicles in accordance with Momentum’s Operational Manual.
• Safely operates Momentum vehicles on surface streets, freeways, highways, and interstates in compliance with DMV and Highway Patrol protocols.
• Operates vehicle lift for people supported, securing wheelchairs with a 4‑point securement.
• Assists with moving people in and out of wheelchairs, automobiles, pool, etc.
• Provides support for program activities, community-based activities, communication, feeding, mobility, toileting, hygiene, and other activities of daily living.
• Engages in program activities including computer class, art class, sensory activities, and gardening.
• Performs other duties as assigned to meet department and/or business needs.

• A high school diploma, GED, or equivalent; required.
• Minimum of two (2) years of related work experience; required.
• Experience working with individuals with developmental disabilities; preferred.
• Must be able to read, write, comprehend, and communicate effectively in English.
• Must have three (3) years driving experience.
• Valid and current proof of legal right to drive in California; required.
• Current DMV driving record printout; required.
• Must be able to participate in pool therapy and aquatic activities.
• Successfully pass all post‑offer, pre‑employment screening requirements, including criminal background check, drug screening, physical exam, and proof of eligibility to work in the United States.
• Ability to run, push, pull, bend, squat, kneel, walk, stand and sit for long periods, and lift up to fifty (50) pounds.
