Brief Description of Principal Activities
Multifaceted intervention, prevention, mental health, and housing assistance is offered through street outreach and 9 service sites throughout Los Angeles County. St. Joseph Center also provides on-site services at five permanent supportive housing developments, and will provide services at an additional six more sites in the coming year. St. Joseph Center’s total holistic approach to services provide our clients with the help they need to regain their stability.
Job Description
Job Summary
The Case Manager serves as a team member with several social service professional and paraprofessional staff ensuring the delivery of quality care in accordance with the rules and regulations established by the Los Angeles Department of Mental Health for contracted agencies while adhering to the mission of St. Joseph Center.
Essential Duties & Responsibilities
The Case Manager’s key areas of responsibility include:

- Provide targeted outreach, engagement, and education activities to Latino individuals and families in order to increase awareness regarding overall mental health and health issues.
- Perform routine case management activities while working with other program staff providing services to clients.
- Help prospective clients gain access to needed community resources and support.
- Assist in establishing for services as defined by agency and funding sources.
- In conjunction with other staff, coordinate activities, recruit participants, arrange event logistics, and provide staff support.
- Facilitate therapeutic groups, including working alongside with other staff to develop and implement curriculums
- Assist and explore individual rehab sessions by providing coping skills to help support clients with mental well-being.
- Participate in regular Latino ISM team meetings.
- Document all advocacy and case management activities as required by agency and/or funders.
Qualifications
QUALIFICATIONS
Knowledge, Skills & Abilities:
- Fluency in Spanish/English language required.
- Interest in immigration issues and/or family systems is a plus.
- Must be highly motivated and a self-starter.
- Must have the ability to communicate with and relate to a diverse group of people including clients, community, and other staff.
- Must have excellent administrative and organizational skills.
- Experience providing mental health and substance use case management services in a community setting with at risk population, experience in Hispanic culture is preferred.
- Bachelor’s degree from an accredited college or university in social work, human services or a related field.
- This position has no direct supervisory responsibilities.
Skills:
- Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and widely supported internet browsers.
- Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
- Valid automobile liability insurance.
- Travel to meet clients may be required.
- Transport clients using company vehicle as needed.
