Compensation
- The starting rate is $30 PER HOUR.
- Mileage will be compensated for commuting beyond a 20‑mile radius of the nurse's residence.
Essential Job Functions / Responsibilities
Coordinate home health care services by managing and overseeing individual home health needs on a case‑by‑case basis.Perform duties consistent with the Vocational Nursing Practice Act.Implement safe, therapeutic oversight of the patient's care with overt needs in supervised/controlled situations as initiated in the Plan of Care in consultation with the visiting nurses and the interdisciplinary team.Utilize resources (patient, family, staff, outside personnel, agencies/organizations) to contribute to the Plan of Care.Maintain current knowledge and skills for documenting care to meet regulatory and third‑party payer requirements. Prepare documentation and clinical/progress notes. Documents must be clear and concise, using proper notation and agency abbreviations. Submit and coordinate all documentation within the timelines established by the Agency.Explain medical guidelines, processes, and procedures to the patient, patient's family, medical providers, and/or caregivers. Document all responses for charting and teaching purposes.Notify/continuously update the patient’s attending physician, responsible staff, and other involved professional persons of patient condition changes. Any changes that suggest the need to modify or develop a plan of treatment or plan of care are considered "significant changes."Record the client's progress, including physician follow‑ups, home visits, and other notable interactions.Evaluate the client's progress periodically and make adjustments as needed to improve outcomes.Regularly attend and participate in scheduled case conferences, staff meetings, and Agency in‑services.Participate in appropriate continuing education as may be requested and/or required by your immediate supervisor. Personnel are expected to accept personal responsibility for other educational activities to enhance job‑related skills and abilities.Attend mandatory educational programs. Attend and/or participate in Clinical Record Reviews and other quality improvement functions as directed by supervisor.Follow up with active and discharged clients to ensure they are satisfied with the services, all requirements are met, and they are in good physical/mental health.Maintain/confine patient and Agency information confidentiality at all times.Provide technical quality assurance of all charts and reporting to the responsible party for corrections.Spanish speaking is preferred but not required
Position Requirements - Active nursing LVN/LPN license
- One year of nursing experience (If new grad, training, and oversight may be an option)
- Current physical, which includes negative TB test or X‑ray.
- Current CPR certification
- COVID Vaccine (As per CDPH requirements, all healthcare providers must be fully vaccinated)
- Must be able to walk, bend, stoop, and lift objects weighing up to 25 lbs.
- Active legal resident status (Passport, Work visa, Green card, etc)
