Position: Registered Nurse (Los Angeles) Home Health

Registered Nurse (Los Angeles) Home Health

Omega Home Health Care Services, Inc. is seeking a qualified Registered Nurse to provide high quality patient care in a Home Health setting throughout Los Angeles County. The Nurse will assess patients and families, develop and implement care plans, and coordinate with other health professionals.

Pay Range

$65.00/hr – $125.00/hr (office based, on the road)

Responsibilities
  • Complete initial assessment of patient and family to determine home care needs.
  • Provide a complete physical assessment and history of current and previous illness.
  • Regularly re-evaluate patient nursing needs and update care plan.
  • Initiate plan of care and make necessary revisions as patient status changes.
  • Use health assessment data to determine nursing diagnosis and develop a care plan.
  • Administer medications and treatments as prescribed, and provide health care instructions.
  • Counsel patient and family on nursing and related needs and discharge planning.
  • Prepare clinical notes and communicate with agency regarding patient status.
  • Coordinate care plan with community health‑related persons.
  • Ensure arrangements for equipment and services are available and supervise home health aides.
Qualifications
  • Registered Nurse licensed to practice in California (current licensure required).
  • Minimum two years’ experience in home health or public health nursing (preferred).
  • Ability to perform lab draws, wound care, and administer IVs (PICC, central, midline).
  • Tracheostomy management experience.
  • Graduate of a National League for Nursing accredited nursing program.
  • Current CPR certification (required).
  • Licensed driver with insured automobile in good working order (required).
  • Strong observation, problem‑solving, verbal and written communication skills.
  • Self‑directed and able to work with minimal supervision; capable of supervising home health aides.
Physical Demands

Regularly required to walk, stand, lift or move up to 50 pounds, and perform repetitive reaching, stooping, bending, kneeling or crouching.
