Job Description Overview

The Licensed Vocational Nurse (LVN) reports to the Nurse Manager and is responsible for managing the individualized patient care by promoting and restoring patients  health through the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients.

Essential Functions and Responsibilities

  • Provides interdependent functions that overlap the practice of medicine that require standardized procedures; follows appropriate guidelines in accordance with the organization’s written policies.
  • Processes CMR reports to county for reportable (+) STD’s.
  • Assists with Prior Authorizations.
  • Reviews patient labs.
  • Triages patient phone calls, portal, and voicemail messages.
  • Administers medications/ draws blood samples/starts IV access (if certified) as appropriate.
  • Supervises workflow of Medical Assistants under the direction of the Nurse Manager.
  • Ensures clinic runs efficiently and on time.
  • Assists with patient outreach for PrEP and PEP patients; ensures that patients follow up with Prevention Coordination care team and that future appointments are placed.
  • Operates within scope of practice per CA Nurse Practice Act.
  • Demonstrates an in-depth knowledge of medical and clinical procedures and terminology.
  • Performs appropriate assessments and treatments per nursing care plan, or as ordered by medical provider, in an accurate and timely manner.
  • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
  • Promotes patient s independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
