Mon‑Fri with on‑call rotation one weekend a month.

The RN Case Manager, Home Health at Accent Care will provide patient‑centered nursing care and case management for home health patients in the Santa Clarita & San Fernando Valley coverage area. • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse (RN) in the state of agency operation.
• At least one year of experience as an RN.

• Licensed to practice as a registered nurse in the state of agency operation.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
