Job Description Wellness Visit Nurse Practitioner.
Responsibilities- Conduct in-home health assessments for Medicare patients.
- Perform comprehensive, face-to-face Health/Annual Wellness Visits.
- Reinforce preventive services and identify gaps in care.
- Provide consultative assessments and diagnosis.
- State licensed and board certified Nurse Practitioner with NPI.
- Must consistently provide a minimum of 10‑16 hours weekly (availability 7 days a week, 8 AM to 8 PM).
- Must be able to travel within the region (Mileage paid).
- Flexibility – design your own schedule with weekend visits as an option.
- No prescribing or treatment; simply performing consultative assessments/evaluations and diagnosis.
- Credentialing services and paid training.
- Liability insurance.
- Scheduling of appointments with Medicare members.
- Web tracking and scheduling tools.
- Ipad tablet/laptop provided.
- Mileage reimbursement.
