we are seeking a skilled Registered Nurse (Home Health RN) with a passion for delivering personalized, in-home healthcare. As part of our team, you will develop and implement patient care plans, provide hands‑on clinical support, and ensure the comfort and safety of our senior clients. This role is ideal for a proactive nurse dedicated to uplifting the lives of our patients.

• Conduct comprehensive clinical assessments, monitor vital signs, and document patient progress.
• Administer medications, treatments, and wound care following established guidelines.
• Collaborate with physicians, caregivers, and family members to coordinate patient care plans.
• Provide patient education on managing chronic conditions and maintaining overall health.
• Ensure compliance with safety protocols, infection control measures, and regulatory standards.

• Valid RN license in the state of California.
• Experience in home health, geriatric care, or related field is preferred.
• Excellent communication, clinical judgment, and problem-solving skills.
• Proficient with electronic health records (EHR) and care documentation.

Each day blends clinical care with care coordination. You'll perform comprehensive in‑home assessments, develop and update care plans, administer treatments, coordinate with physicians and family caregivers, and educate seniors on managing chronic conditions. You'll work autonomously while being part of a tight‑knit interdisciplinary team.
