As a Pain Management Nurse (Home Health), you will assess patients’ pain levels, implement strategies to alleviate discomfort, and educate families on non-pharmacological interventions. Your expertise ensures that seniors can manage chronic pain effectively, maintaining dignity and independence.

• Evaluate pain sources, intensity, and impact on daily functioning.
• Collaborate with physicians to adjust treatment plans and medications.
• Educate patients and caregivers on pain relief techniques, exercises, and relaxation methods.
• Monitor patient progress, document outcomes, and modify interventions as needed.

• Registered Nurse license in California.
• Experience in pain management, palliative care, or home health nursing.
• Excellent communication, empathy, and patient education skills.

Each day blends clinical care with care coordination. You'll perform comprehensive in-home assessments, develop and update care plans, administer treatments, coordinate with physicians and family caregivers, and educate seniors on managing chronic conditions. You'll work autonomously while being part of a tight-knit interdisciplinary team.
