LVN MDS Nurse – Full-Time, Monday–Friday Days | $40–$48/hr | West Los Angeles | Oceana Healthcare Center

We’re looking for an LVN MDS Nurse to partner with our experienced RN MDS Coordinator. You’ll help us capture an accurate picture of every resident’s care across our subacute, long-term care, and skilled populations.

What you’ll do

• Support the RN MDS Coordinator in completing timely, accurate MDS 3.0 assessments for subacute, long-term care, and skilled residents

• Track assessment schedules and help ensure all OBRA and PPS deadlines are met

• Review clinical documentation in Point Click Care  (PCC) and work with nursing and the interdisciplinary team to make sure it supports accurate coding

• Take part in care planning and IDT meetings

• Help identify documentation gaps and communicate with clinical staff to resolve them

• Support quality measures, reimbursement accuracy, and survey readiness through careful, compliant MDS practices

What you’ll need

• Active, unrestricted California LVN license

• Prior MDS experience (required)

• Working knowledge of the RAI process and MDS 3.0

• Experience with Point Click Care  (PCC) preferred

• Strong attention to detail, organization, and time management

• Clear, professional communication with nurses, therapists, and the IDT

• Ability to work fully on site; occasional weekends may be needed

Schedule

• Full-time, Monday through Friday

• Flexible start between 8:00 and 9:00 AM; end time between 4:30 and 5:30 PM

• Occasional weekend work as needed

• This is an on-site day position. It is not remote, hybrid, or a PM shift.
